リベート申込み方法/ How To Apply For The Rebate
紹介者がいない方もご応募頂けます。You do not need a referral to participate.
Step 1. 現視聴者は、申込み用紙の「紹介者」の欄を記入し、新規加入のお友達にお渡し下さい。新規加入者の方で紹介者がいない場合は新規加入者の欄のみご記入ください。
As a current TV JAPAN subscriber, please complete the “Rebate Application Form” and give it to your referral friend.　If you do not have a referral, please complete “New Subscriber” information only.
Step 2.

紹介された新規加入者は、2008年10月１日から2008年12月31日までのキャンペーン期間中にDISH Networkまたはケーブルテレビ、もしくはＩＰＴＶ会社を通じてテレビジャパンの申し込み手続きを終了してください。（DISH Networkの加入手続きはテレビジャパンでもお手伝いします。ケーブルテレビ、または電話会社のＩＰＴＶでご視聴の場合は、直接ご利用各社にお申し込み下さい。）
Your referred must subscribe to TV JAPAN between October 1, 2008 and December 31, 2008 to qualify for this promotion. All orders must be placed during the campaign period.
Step 3. テレビジャパン視聴開始後、本申込み用紙の「新規加入者」の部分を記入し、ご利用のサービス供給会社から届く最初の視聴料請求書コピー（テレビジャパンの視聴料、名前、住所が記載されている部分）と一緒に、2009年1月31日必着でテレビジャパンに郵送して下さい。
After subscription and/or installation of TV JAPAN, please mail the “Rebate Application Form” with the original proof of subscription (copy of first months bill from DISH Network, cable company or IPTV provider indicating your name, address and TV JAPAN subscription) by January 31, 2009 to TV JAPAN.
Step 4. 2008年12月31日までにリベートキャンペーンにお申し込みいただく新規加入者と紹介者は自動的にプレゼントキャンペーンへエントリーされます。 詳しくは商品詳細と応募規約をご覧ください。
