新規加入者は2009年6月29日までにテレビジャパンの申し込み手続きを終了してください。
Please start subscribing to TV JAPAN by 6/29/2009 to qualify for this promotion. All orders must be placed during the campaign period.

Step 2.

テレビジャパン視聴開始後、本申し込み用紙の「新規加入者記入欄」の部分を記入し、ご利用のサービス供給会社から届く最初の視聴料請求書のコピー（テレビジャパンの視聴料、名前、住所が記載されている部分）と一緒に、2009年7月27日必着でテレビジャパンに郵送してください。後日テレビジャパンより、視聴料一か月分（上限＄30）のチェックをお送りいたします。
After installation of TV JAPAN, please mail this “rebate application form” with the original proof of installation (copy of your first month’s bill from DISH Network or local cable or IPTV company indicating your name, address and TV JAPAN subscription) by 7/27/2009 to TV JAPAN.
